Cargill Consulting’s 2009 Cheetah Awards

1. List All Team Members. Include grade and high school. Identify the team leader(s) and contact information for the leader(s) including phone number and e-mail.

2. Complete 3 Boxes. List the organization, title, and roles and responsibilities.

3. Summarize project. Write no more than 5 sentences describing the project.

4. Include Pictures. Include at least one picture of your team members only. Send up to 5 additional pictures of your team doing the project. These pictures can be submitted by e-mail or by postal mail.

5. Contact Information for Organization. List contact information for a representative from the organization where the team did the project. This will allow us to contact the organization about your project.

6. Deadline: March 1^st. The application must be postmarked or e-mailed to chad@chadcargill.com by March 1^st.

Requirements: Only teams of 3 will be accepted for the 2009 Cheetah Awards contest. All 3 team members must be in the 9^th – 12^th grade during the 2008-2009 school year. This contest is only for students who attended Chad Cargill’s Test Prep and Character Building Workshop; therefore, all three team members must have attended. The project must have been completed during the 2008-2009 school year. All pictures become property of Cargill Consulting, Inc. and will not be returned.

Results: 10 teams will be named as finalists. The team leaders will be contacted for a phone interview. An additional interview may take place in person at your high school if the committee deems necessary. All 10 teams will be featured on our website and through other media outlets. Each member of the winning team will receive a brand new laptop computer. The Cheetah Award winners will also be featured in Chad Cargill’s workshop during the 2009-2010 school year.

Release: A legal release form must be signed allowing Cargill Consulting, Inc. to use your pictures and information for concerning this contest. If you are under 18, a parent or legal guardian must sign the form. Each member of the team must complete the legal release. These must be submitted through postal mail postmarked no later than March 1^st.

The pictures you provide to Cargill Consulting, Inc. will be used with your permission. The legal release must be signed and included with your entry for the 2009 Cheetah Awards. This release only applies to the pictures you send for the 2009 Cheetah Awards.
